Scope of Services

• Affliate with the State of Hawaii Expanded Adult Residential Care Homes • Assist with location of appropriate residential care facility placement for clients that require ICF level of care. • Perform comprehensive assessment of client, including but not limited to a review of medical, nursing, social, mental, behavioral, recreational,  and spiritual needs. • Development of client centered care plan. • On-going evaluation and monitoring of client status, caregiver skills & competency, and quality of services provided. • Promote and facilitate continuity of care and appropriate integration and utilization of services. • Coordinate caregiver training, hospital discharge, home transfers, and other services as appropriate. • Provide health related in-services to residential care homes and foster homes.
